Recommended Prerequisites

Human Anatomophysiology, Clinical Biochemistry, Human Physiopathology, Pharmacology and Pharmacotherapy.

Teaching Methods

The teaching will be carried out through lectures of matter, with the timely collaboration of pharmacists specifically related to the practice of Clinical Pharmacy.

The evaluation will be made by the means of a final exam: theoretical evaluation (16/20 points) and theoretical and practical evaluation (4/20 points).

The student must obtain a minimum of 6.4 points ​​in the theoretical part (maximum 16 points) for approval.

There is no place to carry out subsequent oral exam to the written test.

Learning Outcomes

Knowing that Clinical Pharmacy is defined as that area of Pharmacy concerned with the rational medication use, this curricular unit aims to prepare pharmacists to integrate a health care team, playing its unique role in taking patient care, particularly by improving medicines-related health outcomes for individual patients.

Syllabus

Theoretical component: Clinical Pharmacy as a field of Pharmaceutical Sciences. The clinical method. Pharmacotherapy and the patient’s health. The issue of adherence to therapy. Clinical pharmacists services: medication reconciliation, medication review, preventable drug-related morbidity indicators, pharmacotherapy follow-up. Polypharmacy. The elderly and the medicines. Individualization of therapy (dose adjustment in renal and liver failure, clinical pharmacokinetics). Medication errors. High-alert medications. Intervention of the clinical pharmacist at different levels of health care (primary care, intermediate care, acute care).

Theoretical – practical component: consolidation of knowledge in the theoretical component by solving clinical cases.
